I've met some transplants who've had 0 or 1 matches and are doing well, and seen some perfect matches back in dialysis. Although, you will definitely need more immunosuppression medicine along the way. Just a thought, I dont know how quickly you need the kidney and how bad your brother's blood pressure is. Coz it may be possible to put him on exercise and diet and stop smoking (if he does) for a while and get the BP down, which is easily done. Just my 2 cents.
Generally, the better match provides a better probability of not only survival, but also less problems along the way and longer 'life span' of the kidney. But there are so many other factors like how you take care of yourself and what happens post transplant (ie. infection,etc).
