It sounds like your code did not program the flash correctly. Did you remember to use the program-verify feature of the programming algorithm and the erase-verify feature of the erase algorithm. If the contents were not versified correctly as given in the algorithm the data retension cannot be assumed to be that specified in the hardware manual. Also check that the re-program and additional re-program data is being correctly calculated. Apps note 125 from the link below should help.
