I am not sure why Dr. goodyear used the PHS vs the UHS. The PHS is a heavy weight polypropylene mesh and will contract and harden over time. Neurectomies don't get rid of all the pain. If you don't get better, consider removal and a shouldice may be possible, but with the previous surgery, you may need a biologic mesh, ePTFE, or a lightweight polypropylene to complete the repair.
Most PCP's don't understand hernias and refer to the surgeons they know. It has little to do with the skill and expertise of the surgeon. I fight this battle all the time, but I do have a large core of referring doctors. Many of my patients come from former patients that refer friends, etc.
