there is really no reason to make a separate incision in the scrotum to do a vasectomy. During surgery for the hernia repair, your surgeon has easy access to both vas deferens from the inside and it takes about 60 seconds extra to do a vasectomy at the time of a hernia repair. I don't think you will have much more pain with a scrotal incision, but why make one if you don't need to.
