1. a lipoma can recur, but almost never does
2. the design of mesh (i.e. heavyweight) leads to contraction
3. mesh is almost never removed during a repair for a recurrence. A lap recurrence is best approached via open techniques.
4. biologic mesh may eventually replace synthetics once it comes down in price and is shown to be safe and effective for groin hernias.
5. There is virtually no role for the robot in hernia surgery unless it helps the surgeon market his practice.
