It is important that patients like you, do their homework prior to an important operation like a hernia repair. A scrotal hernia involves a defect at the internal ring and a hernia sac. The defect is in the Myopectineal Oriface. The UHS or PHS covers this defect with a FLAT sheet of mesh. It is NOT a plug. Plugs are BAD! An open repair under local anesthesia should be possible unless your hernia is very, very large. Then, a regional or general anesthetic would be needed. Check the listings of physicians at the American Hernia Society (http://www.americanherniasociety.org) for a referral, but if I were you, I would see someone in Dr. Gilbert's group in Miami (Hernia Institute of Florida). He invented the system and they have done close to 15,000 repairs! Dr. Gilbert is now retired from clinical practice. Good luck.
