Assuming the prior three procedures have been done "open", the laparoscopic approach is the one that you MUST have done. This approach offers access to the hernia defect via virginal tissue planes rather than violating the scar tissue in the groin from prior operations. With the laparoscopic approach, the recurrent hernia will be well seen and the complete myopectineal orriface can be properly covered with mesh with almost no chance of recurrence. The surgery, however, is not to be done by the casual laparoscopic surgeon. It should only be performed by an experienced laparoscopic hernia surgeon who has performed previous recurrent hernia repairs.
