It's always been my feeling that Venger and Kareena are his biological children. Dungeon Master may be short, but that doesn’t make him some other strange species. He is just a unique human being of small stature, with a tendency for half-truths. The evidence suggests that both he (at the garden in Tardos) and Venger (travelling to the Hall of Bones, etc.) can take any form they choose; and maintain it with no obvious effort. I'm assuming that Zandora could do the same. Who is to say that what they appear to be to the Young Ones is who they truly are?
What I would offer is that Venger chose his form to be what it is. What better way to command the fear and respect of his underlings; and the people of the Realm than to be something dark and scary? I’d like to think also that it mirrors his spiritual corruption. He is as much a monster within as without, even if he has some twisted code of honor.
Kareena is no special case either. She is a vain and selfish woman in the body of a teenage girl, because that’s who she chooses to be. She uses her “helpless” appearance as a normal girl to trick Sheila into doing what she wanted, because she likes being manipulative. If you watch her closely, she has fangs like Venger in a few scenes, and so her true nature is revealed under all the trappings of false innocence.
