You mean the guy in the background in this picture?
This came up once before on this board, and someone suggested, and many people agreed, that the man in the background is a cameo appearance by a younger Stan Lee, and that the kids are young versions of the Fantastic Four. It makes sense considering the cartoon's connection to Marvel, and the caricature looks more like him than Gary Gygax or anyone else D&D-related. How about it? Does anyone have a different theory?
As for never seeing the operator and Out of Order sign mentioned in the Series Bible, keep in mind that what you're reading is the first draft that was proposed to Marvel. It underwent many, many changes and revisions before becoming the cartoon we know. (Note the word DEVELOPMENT on the image of the title page.) For instance, there was an entire paragraph about the red dragon Rogull, who never once appeared in any of the episodes because he was later revised and replaced by Tiamat, who, you will notice, was never once mentioned in the series bible to begin with. At some point, someone decided the intro sequence needed to be reworked from the original draft, and it became what we see now.
