Sorry to spoil your worthy mission, but you need to check with the local health authorities. In most places you cannot take home-cooked meals into a licensed facility (at least in the US).
Also, check with the facility dietician for the best approach to adapt your meals to those with special diet needs.
Why can't you cook the meals in the home's kitchen? Then you don't have to worry about transporting potentially hazardous food.
