IMO too awkward and too many unnecessary words, Gerry....not plain language at all. Unless otherwise stipulated by your SOS, you could use:
STATE OF
COUNTY OF
On October 28, 2009, before me, Mary Roe, a notary public, personally appeared John Doe, who acknowledged the foregoing instrument and {optional wording: "to be his free act and deed"} provided satisfactory proof of identification.
