This is hypothetical.
Since notaries in my state may use any wording they want for acknowledgments, as long as all the necessary information is there, I decided to try to write an acknowledgment in plain language that describes what the notary knows, without implying anything the notary doesn't know. It's long because it doesn't assume any meaning of words that lawyers know about but the general public doesn't.
I WOULD NOT USE THIS FOR REAL, but I'd be interested if anyone thinks I left something out, or put something in that shouldn't be there.

Today, October 28, 2009, a man using the name John Doe personally appeared before me, Mary Roe, a Vermont notary public. He
presented SATISFACTORY PROOF that he was entitled to use the name John Doe, and he presented the foregoing instrument, which names contains the name John Doe, bears a signature which reads John Doe, and bears a
personal seal consisting of the printed letters "L.S." He acknowledged to me that he signed the instrument, and adopted the printed letters "L.S." as his seal, of his own free will for the purposes set forth in the instrument.
