I'm not a California notary, but if someone asked me to do something with that form, my response would depend on exactly what I was asked to do. I notice that the form says, in the part completed by the employee, "I attest, under penalty of perjury, that I am. . ."
Since the exact wording of an oath or affirmation is not specified in my state, I would consider that to be acceptable wording for an affirmation and I would have no problem attaching a loose certificate indicating I had administered an affirmation to the potential employee.
Whether that would actually satisfy the requirements for filling out the form is not my concern.
So, what exactly did your would-be client ask you to do?
