Spousal Documents

Have a problem.
We have a non-borrowing spouse that is listed on the documents with her married name ie. Rooney but her maiden name is Swartz. She has no ID with the name Rooney on it as she has never changed her maiden name. Which documents would you have her sign and HOW?
