Posted by John Biggers If the recipient decides to contact your family they can, but usually anonymously through the OPO. To contact the OPO and find out exactly what their policies are, go to the UNOS website and then to the Search page, select “Organ Procurement Organizations" and enter the state; find the OPO that covers the hospital where the donation took place. It will be up to the recipients to write and try to contact you. (In my case, I had to write an anonymous letter to the family, forward it to the OPO, then they make it available to the family if they want to read it; if they want to contact me after that, they can, or they can ignore it. If they choose to do so, they can identify themselves. My donor family contacted me, and we have exchanged several letters and thoughts.) John

All transplant centers differ a little bit in the way they handle family to recipient (and recipient to family) contact: the main goal is to ensure confidentiality to either party if they need or want it.
