For a while I was feeding a recipe I got off of the BalanceIt website at:

It was called "BARF-like Chicken For Dogs" (the site has a bunch of different recipes, but I liked this one because of all the veggies). They charge for each recipe, but you can usually use a code to get one free (I think I used "VIN"). Their recipes also require the addition of a vitamin/mineral supplement, and they give you the option of buying theirs, or making up your own out of human-grade products according to the formula they provide.
Recently I switched him to a premix from The Honest Kitchen called "Preference". It's meant to be mixed with raw meat (variety is better) and already has the proper blend of vitamins/minerals in it. Although he's previously disliked being offered raw, in this mixture he really likes it. And his kidney values (he's 13 years old) are coming back down to where they belong, so I'm happy!
I did buy Donald Strombeck's book on Homemade Diets for Cats and Dogs, but was less than impressed with the kidney diets it had. Lots of people speak highly of it, however.
