funding and services for the disabled

I'd be interested to know what grant resources (outside of FC pubs) are available in your libraries specifically for individuals with disabilities, or for nonprofits providing services to this group. Our titles are posted through the New York library home page, and we have a small circulating collection for those in the metro area who can't get to our physical space. If anyone has a wonderful bibliography to share, I'd love to see it. Since we don't track government grants, helpful hints on navigating the maze to help callers would be appreciated. The thorniest cases are those who need rent money, transportation, etc. We get calls from all over the country and need to make better referrals.
