Survey to get feedback

Hello,
I am the beginning stages of developing a survey/questionnaire for those who use the FC database at our CC to find out if they found a grant, if they completed a grant application, and if they received funds.
Do any CC supervisors use questionnaires to obtain feedback about their library? If so, do you have any examples of your questionnaire or survey?
