My guess is the doctor practices at PVH, so they can have general exams there, but the real work is done at his clinic outside the hospital, the better chance Jake won't be lurking around. So they could still be trying to do AI, it's just that Amanda is at the end of her fertile 48 hours for the month when David's car "breaks down." IOW, he never intended for them to do AI, because he wants Amanda, he wants their new baby and Trevor and them to be a family, and he wants to torpedo her marriage!
