When you lay on the bench and reach up to grab the bar do you reach so far back that your upper arms goes past perpendicular to your body. Try setting your grip with your throat under the bar and dont slide out any farther than triceps prepedicular to the floor. If you slide out to far or reach back for the bar the shirt will ride up even with a belt on
