Need help identifying problem

Any help with this would greatly be appreciated:
A-1 Bentgrass seeded in early May (Ohio) on a 50/50 sand clay mix. Looked great after 3 weeks.......but has made a turn for the worse. I now have several brown streaks throughout the green in a sporadic manner. I have been trying to identify the problem but can't pinpoint it.
Today I noticed a mossy like green substance on a few bare spots that haven't grown in and found this same substance under the browned grass. Anyone know the possible problem from these few clues?
