Many things could cause this but one of the first things I check with these symptoms is to make sure that the fuel tank is venting properly. A simple way to check is to drive until it starts to crap out, and then quickly pop open the fuel cap. If you hear a swishing sound, and it restarts & runs properly for awhile, it is not venting & is building up a vacuum as fuel is burned off, increasing the load on the fuel pump.
