I had one given to me by Orthopedist for my knee. After I lost weight I didn't really need it but when I looked it said I met the guidelines for COPD which were FEV1 less than 1 liter so I didn't turn it in. They had given me a permanent one for my knee so it just comes to me every two years. Sometimes they give a temporary one but he told his sec'y to give me a permanent one. I had mixed emotions at the time. I don't use it if I can find parking fairly close. Seems to me everybody I know has one and yet they can walk for miles when they are traveling and sightseeing. Also, have some people that do a lot of odd jobs around where I live and they have one so it isn't hard to get. I had to convince my parents in their 90's to get one even after my Mom had a couple strokes and my Dad had a catheter and colostomy full time. Different generation. They were the tough ones.

Your doctor should have the form that you need to fill out. Just send it in to your state DOT (or wherever they direct you.

Sure have, but it's your Doc who signs the application for you. Next visit, just ask, as they usually have the forms available or you could bring a blank from Motor Vehicles. I chose the placard so that we have the flexibility of whether or not to use.

Absolutely no problem in Nevada. They give you a lifetime one if you are diagnosed.
