She had home health before she went in, so they will do a resumptioin of care when she comes out, providing she is dc'ed from the hospital or rehab within 60 days of the first day the nurse came to visit her at home. If it's within the 60 day window, the same agency will normally resume care, unless the patient requests another agency. In which case, the original agency will have to be notified before the new agency comes in as Medicare will show the first agency as an open case. This is where things can get a little hairy, but it's also the perfect time for her doc to research which agency has the telemedicine setup I mentioned above. Sounds like it would be a good thing for her to have.
