At first I thought they were just talking about accidental which is just that "an accident" The patient knows how to take it but messes up and takes it by mistake. The second paragraph in Gary's post sounds more like people are doing it more than just by mistake because they talk about patient education and labeling which is really not going to help if someone just makes a mistake. I am not sure how education will help with "swallowing accidents" Most of us realize the minute we did it that we made a mistake.
