Well the BJA did a meta analysis of available research and found no difference in outcome, the only study that found difference in outcome was disowned by its authors. As for gold standered why is it assumed, because that is what it is an assumption that MD anesthesia is any safer then nurse anesthesia? or that the care team is more effective, I may buy the two heads but does it make a difference if one is an MD or how about 2 MD's? or two CRNA's? The problem with the care team is that it assumes the superiority of MD anesthesia from the outset regardless of the experience of the practitioners, there is simply no scientific basis for this cultural prejiduce,and that is all it is a cultural prejudice.
