Most people recommend a litter box with high sides for cats with this disorder, so they can lean against the sides. Timmy did not like the high sided box so I switched to a shallow one that he can just step into. It is in the corner of the laundry room so he has two walls to lean against and two open sides to walk into the box.
He does occasionally fall down while in the box and has even fallen out of it, resulting in a clean up for me. Twice I found him laying on his side with his head on the floor and hips propped up on the edge of the box, with a good water spout going. Apparently he can't just shut it off when he falls over.
Since he relys on his front legs for balance, he can't dig or bury, so I have to scoop the box often to keep the surface clean. Otherwise, he is likely to fall on top of a poo pile on his next trip. He does get baths sometimes.
